Covers have abrasion on rear panel, spine and corners; cracked at bottom front joint. St. Mary of Nazareth Hospital was established in Chicago at the end of the 19th century-- initially to care for the underserved population of Polish Catholics immigrants. Former owner last name on front endpaper, and a few notes on rear page set aside for autographs in the rear. A note thanking sponsors says that this is the third yearbook. 122615B
